INSURANCE
I accept Asuris and Aetna, Kaiser (WA), Premera, Blue Cross Blue Shield, Regence (WA) and First Choice Health as in-network provider. For all other insurances, I am an out of network provider and will provide you with a superbill for you to be reimbursed by your insurance company (you pay upfront). Insurance rates are available upon request.
A note on insurance: I am happy to accept insurance however please be aware that in order to accept insurance I must provide you with a diagnosis that will become part of your permanent medical record. This may have implications for “pre-existing conditions” for future insurance needs etc. I may also have to release your documents to your insurance company for review. If you are using out-of-network benefits, it will be in your best interest to call your insurance to see if they cover telehealth services, how many visits you are covered for and what they will reimburse you for. I cannot guarantee reimbursement by your insurance.
Payment is required at the time of service via all major credit card carriers. HSA/HRA cards (for video services only). I will provide you with a client portal to provide payment. For future sessions your card can be kept on file and billed automatically (through a secure client portal called Simple Practice). Cancellations are to be within 24 hours or your session will be charged in full.
For out of network provider insurance coverage the session is paid privately upfront and I provide you with a “super bill” for you to submit to your insurance for reimbursement to you. This applies to video sessions only. You are responsible to understand your benefits, referral requirements, reimbursement rates and coverage prior to initiating treatment and submitting for reimbursement. I cannot guarantee reimbursement by your insurance carrier.
